Support at Home

Oxalis Community Services has partnered with Trilogy Care, a registered aged care provider, to deliver Support at Home services that help older Australians remain safe, independent, and connected to their communities while continuing to live in their own homes.

Trilogy Care focuses on assisting you to self-manage your care plan and funding, while Oxalis Community Services focuses on delivering the services you need to stay at home for longer.

By combining strong care coordination with hands-on support, this approach ensures you receive the right level of care, with the flexibility and choice to access services in a way that works best for you.
